Setting up the appointment booking link

The appointment booking functionality allows practices to add a booking link to messages sent to patients. Patients can click the link, confirm their date of birth, then submit their preferences/availability for an appointment. Their answers are emailed to the practice, along with a list of all tasks the patient should be booked for.
